Patients visit the dentist for a variety of reasons. On your first visit, our objective is to work together to see how dentistry can best serve you and your goals.
During your first appointment, which can take 60-90 minutes, we will collect and review all the information needed to provide a personalized treatment plan that addresses any concerns. This includes your medical and dental history forms, digital x-rays, photos, and even a digital scan of your teeth. All these diagnostic tools allow us to easily show you what we see.
To save time, please fill out your patient information forms before your appointment digitally. They can be found below.
Things to bring to your appointment:
- Photo ID
- Any insurance card
- Any recent dental records or X-rays from your previous dentist
- Digital copies can be emailed to hello@rinconfamilydental.com
- Most importantly, your smile
